District Nursing
Our District Nurse team is comprised of highly qualified nurses who provide general and specialised nursing and healthcare to clients to improve their health status and retain their independence to continue living at home.
What can be provided?
The District Nursing Service provides assistance with:
- Wound Care
- Medication management
- Assessment of health status including blood pressure and assistance with diabetes management
- Palliative Care
- Post acute care
- Assessments for Domiciliary Nursing Benefits
- Referrals for Aged Care Assessment and other health care professionals
- Education for patients and carers about health care needs
- Co-ordination of care with other health service providers
- Continence management
- Monitor general health
- Blood pressure
- Blood sugars
- Blood collection for Pathology
- Carers support coordination
- Health assessments
- Bereavement management
Who is eligible for care?
Nursing services are offered to anyone requiring home based nursing care. Community members that would like to use the service are required to be assessed. Referrals can be made by self referral or by your doctor.
People who receive benefits from the Department of Veterans Affairs, Workcover or Transport Accident Commission can also be referred to the service.
When will a nurse call?
Following referral to the service a nurse will phone to make an appointment for an initial visit. This visit can take up to an hour. Carers are encouraged to be in attendance at this appointment as a plan of care is developed during this time.
Subsequent visits will be made on nominated days, depending on needs. However, a specific appointment cannot always be made.

My Community & Home Care (MCHC)
Are you growing older and worrying about being able to stay at home? Are you living with a disability? Are you caring for a family member or friend at home?
The My Community and Home Care Program at THS is funded to provide a range of services designed to support you, or the person you care for, to stay active and independent and living at home for as long as possible. Helping people stay active, involved and doing as much for themselves as possible is proven to help people stay more independent with a greater sense of wellbeing. This is the approach we take in working with you.

The Commonwealth Home Support Program (CHSP)
The Commonwealth Home Support Program (CHSP) provides a range of support services to people over 65 (50 years or older and identify as an Aboriginal and/or Torres Strait Islander person) who are experiencing difficulties in managing daily tasks, but who wish to continue living at home.

Veterans Home Care
Veterans Home Care includes Corryong/Walwa areas).
Services provided include:
- Domestic Assistance
- Personal Care
- Safety-related Home and Garden Maintenance
- Respite Care
If you are a Gold Card or White Card holder with low care needs, DVA’s Veterans' Home Care (VHC) program can assist you to continue living in your home by providing you with a small amount of practical help. The VHC Program also assists carers in recognition of the vital role they play in the veteran and defence community.

Home Care Packages Levels 1-4
If you are an older person over the age of 65 years or over (50 years and over for Aboriginal and Torres Strait Islander people) and you have intensive, multiple or complex needs you may require a Home Care Package.

Short Term Support
Short Term Support following an hospital admission or presentation at an Emergency department at a Public Hospital Contact 02 60715 238 and ask to speak with an Assessment Officer or a District Nurse.

Other Food Services
If you would prefer we can help you with preparing and cooking a meal in your home. We can also arrange for you to meet with our Dietitian to discuss your food and diet needs.

Regional Assessment Services
The Regional Assessment Service is for people 65 years and over or 50 years and over for Aboriginal and Torres Strait Islander people.
The Regional Assessment Service (RAS) is a face to face Assessment undertaken by a trained Home Support Assessor to work out your aged care service needs and to refer you to appropriate Commonwealth Home Support Programme services.
To work out if you need a RAS Assessment, you will need to contact the My Aged Care Contact Centre on 1800 200 422 or contact the Home Support Assessment staff at Tallangatta Health Service on 0260 715 239. They will speak with you about your needs and determine whether you are eligible for a RAS Assessment.

Community Room Hire
Tallangatta Health Service has available for hire a Conference Room and The Activity Centre facility.
The Conference Room
The Conference Room has seating for up to 12 persons in a board room seating arrangement. Equipment such as projector screen and video conferencing facilities are available. A small kitchen with beverage making facilities.
The Activity Centre
The Activity Centre has seating for approximately 30 people with a kitchen available for hiring. There is a large flat screen TV, connected computer and DVD equipment which is also available when required.

Red Cross Patient Transport
Red Cross provides patient transport to hospital and specialist medical appointments for people who have no other means of transportation and live within 50km of Wodonga or Tallangatta.
A Red Cross Patient Transport referral is required from your GP or specialist and will be sent directly to the service who will confirm your booking and pick up time directly with you.
This service is run entirely by volunteers and is not a THS run option but is run by Red Cross - North East Patient Transport.
